$157,040/year The University of Vermont Medical Center seeks a Transitions of Care (TOC) Ambulatory Pharmacist Supervisor to work in the Burlington, VT area, and be responsible for overseeing the daily operational workflow of the Meds to Beds program, the transitions of care staff and all associated services.
Job duties include:
(i) developing and implementing operational policies designed to identify and address medication access barriers and liaison with available programs, resources, and members of the care team to help facilitate a smooth transition out of the hospital, promoting discharge therapy adherence and helping prevent non-adherence related readmissions; (ii) implementing the Meds to Beds program in in units throughout the medical center and working directly with unit management to ensure program success and operational efficiency; (iii) overseeing clinical triage and resolution of ambulatory prescription concerns for both the retail and Meds to Beds operations as well as continuous quality improvement program implementation; and (iv) working with the Manager, Outpatient Pharmacy to develop and implement wraparound services. Must have Doctor of Pharmacy degree (or foreign equivalent degree) and minimum one (1) year of pharmacist experience in a hospital-based ambulatory/Outpatient or transitions-of-care setting. Must have experience coordinating multidisciplinary teams, including nursing, case management, hospitalists, and care transitions teams. Must have demonstrated experience coordinating transitions-of-care services, including medication reconciliation, discharge counseling, and high-risk medication review. Must have experience with developing staff training, competencies, or policy/procedure documents. Must have experience managing insurance prior authorization / financial assistance workflows for discharge medications. Must be able and willing to work days, evenings, weekends, and holidays, and be on call on an as-needed basis Must have (or be eligible for) a VT pharmacist license.
