Key Responsibilities:

Conduct comprehensive annual home health risk assessments as part of a Medicare Advantage health plan Review and evaluate patients prior/current medical and medication histories Educate patients regarding medical conditions, prescribed medications, and overall health Identify gaps in healthcare or necessary follow-up, and provide actionable recommendations Complete documentation accurately and timely; no requirement to develop treatment plans or prescribe medications

Required Qualifications:

Active Nurse Practitioner license in North Carolina Certification in Basic Life Support (CPR/BLS) Board Certification through AANP or ANCC Master of Science in Nursing (MSN) degree from an accredited school At least 6 months of recent NP experience preferred
