Position Summary The Hook Up and Bridge (HUB) Program is designed to deliver holistic HIV prevention and care services while linking clients to supportive services. The program emphasizes client choice, wellness, and quality service provision, recognizing the importance of supportive services in overall health and well-being. The STD Specialist supports ACQC's Harm Reduction program by providing low-threshold harm reduction education, rapid testing, and referrals to support services. This role focuses on engagement and support of TGNBI+ (Transgender, Gender Non-Conforming, Non-Binary, and Intersex+) individuals, PWID (People Who Inject Drugs), and people who exchange money for sex. The STD Specialist serves as a trusted community presence and works to increase awareness of HIV prevention services, PrEP/PEP, harm reduction, mental health support, and other health resources. Key Responsibilities Assist with the day-to-day activities of the program. Conducts ongoing needs assessment including a review of characteristics, statistics, and data to help develop or design services that are specific, sensitive, and culturally relevant to the target populations. Conduct and coordinate outreach to target population in the venues, neighborhoods, or areas where potential participants may congregate. Provide individual health education and information on PrEP/PEP. PrEP/PEP referrals and linkage to medical services Conduct community presentations to increase awareness of the program and promote recruitment into program interventions/activities. Delivery of HIV/STIs/HCV counseling, treatment, and referral. Follow up all referrals to assure clients compliance, Maintain accurate and up-to-date client/chart recordings for program activities and council, treatment, referral, interventions. Enter data for all program activities weekly. Submit weekly program and activity progress reporting. Willing to work long hours, evenings, and weekends. Perform other duties as assigned by the Harm Reduction Supportive Services Program Coordinator and/or the Director of Harm Reduction. Qualifications Required Bachelor's degree in social science or health related field and one-year experience, or three to four years work or field related experience. Knowledge of HIV/AIDS, STIs and affected communities. Knowledge of PrEP/PEP Fluency in one or more targeted community languages and/or dialects. Must be culturally competent and able to work with individuals from non-traditional backgrounds in terms of gender identity and sexuality. Must be able to convey sensitivity in a non-judgmental manner. The STD Specialist position is a full-time, non-exempt position, based at two sites: Richmond Hill, Queens, and Woodside, Queens.
