Administrator, Assistant Administrator, Director of Nursing Position Summary At Cavalier Healthcare, the PRN Transportation Coordinator is responsible for providing safe, timely, and compassionate transportation services for residents traveling to medically necessary appointments. This position ensures residents are transported comfortably, assisted as needed before, during, and after appointments, and that transportation services are coordinated in a manner that supports quality resident care and regulatory compliance. Essential Duties and Responsibilities Safely transport residents to and from scheduled medical appointments, diagnostic testing, dialysis, and other medically related services. Assist residents with entering and exiting the transportation vehicle using appropriate transfer techniques and equipment while maintaining resident dignity and safety. Escort residents into medical facilities and provide assistance during check-in and check-out processes as appropriate. Coordinate transportation schedules with nursing staff, physicians' offices, hospitals, and other healthcare providers to ensure timely appointments. Communicate appointment updates, changes, and resident status with the interdisciplinary team. Ensure all required transportation documentation is completed accurately and submitted in a timely manner. Maintain the cleanliness, safety, and readiness of the facility transportation vehicle(s), including routine inspections and reporting maintenance concerns. Comply with all federal, state, local, and facility policies regarding resident transportation, confidentiality (HIPAA), infection prevention, and resident rights. Demonstrate knowledge of and adhere to facility policies regarding fire safety, emergency preparedness, infection control, bloodborne pathogens, Hazard Communication, and workplace safety. Recognize, prevent, and immediately report any suspected abuse, neglect, exploitation, or mistreatment of residents in accordance with facility policy and state and federal regulations. Promote a safe, clean, respectful, and resident-centered environment. Participate as an active member of the interdisciplinary care team by attending care conferences, department meetings, in-services, and other required educational programs. Build positive relationships with residents, families, physicians, vendors, and fellow team members while providing excellent customer service. Maintain professional conduct and confidentiality at all times. Perform other duties and responsibilities as assigned by the Administrator, Assistant Administrator, Director of Nursing, or designee. Qualifications High school diploma or equivalent preferred. Valid driver's license with a safe driving record and the ability to meet facility insurance requirements. Current CPR certification (or willingness to obtain within required timeframe). Ability to safely assist residents with transfers and mobility using proper body mechanics and assistive devices. Strong communication, organizational, and interpersonal skills. Ability to maintain resident confidentiality and exercise sound judgment. Working Conditions Works in healthcare facilities, medical offices, hospitals, and community settings. May be exposed to infectious diseases and bodily fluids; appropriate personal protective equipment (PPE) is provided and must be used. SUMMARY OF OCCUPATIONAL EXPOSURE
Must be able to tolerate a high pace as typical for a nursing facility. Must be able to take and give instructions well and enjoy working with the elderly. Although employee is required to sit for prolonged periods of time, he/she must also be able to tolerate prolonged standing and walking. Must maintain good driving record. Must be covered by automobile insurance. The facility will maintain the vehicle (gas, oil, etc.); however, the employee is responsible for parking tickets, speeding tickets or any other traffic violations. Traffic violations could result in disciplinary action by the facility. In the event of any accident, the employee is expected to fully cooperate with the authorities.
