Summary Description:
Greater Mental Health of New Yorks Care Management Program helps individuals access essential services, such as medical specialists, transitioning from hospital admissions, and securing social supports, such as housing. The Care Manager is responsible for delivering comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Rockland County. This includes assessing, overseeing, and coordinating various needs such as medical, behavioral health, substance abuse, and social services. The role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices.
Key Responsibilities:
The care manager will manage a caseload of clients consistent with industry standards, meet regulatory expectations/ guidelines, and reflect our client population's needs. The exact number of clients may vary based on factors such as case complexity and the availability of support resources. The care manager is expected to maintain a balanced caseload for high-quality care and effective case management.

Provide comprehensive care management and care coordination through face-to-face and telephonic services.

Completes assessments and approves applications for services and entitlementsWith the clients input, creates individualized plans of care Assists in discharge planning during a clients hospitalization and provides follow-up care as neededConsults with team members and other specialistsParticipate in meetings with community agencies and service providers.

Follow established agency policies and maintain professional standards.

Prepare required reports and documentation.

Provide after-hours emergency coverage and crisis intervention as needed.

Demonstrate flexibility with working hours.

Adhere to standards established by regulatory authorities.

Perform other duties as assigned.
Competencies:
Knowledge of behavioral diagnoses and symptomatology.

Familiarity with community resources.

Effective client engagement skills.

Understanding of chronic disease/behavioral health interactions.

Ability to work independently and efficiently.

Proficiency in using Electronic Health Record systems.

Practice in a recovery-oriented, person-centered methodology.

Key Performance Indicators (KPIs): Care Manager is expected to complete CANS-NY assessments at enrollment and annually.

Care Manager is expected to meet program productivity standards.

Care Manager is expected to complete documentation in a timely manner, as per program and agencys standards.
Requirements:
Bachelor's degree in a relevant field or equivalent.

E.g. social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services fieldTwo years of relevant experience or equivalent. A masters degree in one of the listed education fields may be substituted for one year of Experience.

Strong computer knowledge: typing, communication tools, scheduling tools and web browsers.

Proficiency in Microsoft Office suite and database management systems.

Fluency in English (Reading, Writing, Speaking). Fluency in Spanish is preferred but not required.

Valid Driver's License and satisfactory driving record.

The Care Manager will be using an agency vehicle and is responsible for adhering to the Agency Driver Policy.

Maintain New York State Defensive Driving certification within 30 days of hire.

Must maintain pre-employment clearances by applicable regulatory clearances as required.

Legal eligibility to work in the United States without sponsorship.
Special Considerations:
Hours per week, reporting structure, and location are subject to change per program needs.
