Job Description

CASE MANAGER/RESIDENTIAL SUPPORTIVE LIVING
Under direct supervision, the Case Manager for Supportive Living will be responsible for all day-to-day supportive living residential activities The program will focus on harm reduction and provide access to various supportive services, including: ongoing case management; job counseling and job readiness training; linkages to safety net programs and community resources and educational and vocational programs; support for ensuring the educational and developmental needs of children are met; referrals to physical, mental health, and substance use disorder (SUD) treatment as needed and desired by tenants; access to Peer Recovery Coaches; assistance identifying and securing safe, affordable childcare options; healthy eating and cooking education; budgeting; assistance connecting with legal resources; universal skill-building such as coping, communication, collaboration, problem-solving, action planning, goal setting and monitoring, resource management, etc.; among others. Support services offered will be individualized based on the needs of the families in the program.REPORTING RELATIONSHIP
The Case Manager reports to and receives supervision from the Asst. Director of Supportive Living / Residential Services or their designee. DUTIES The Case Manager is expected to: Case Manager determines eligibility and level of need and provides program orientation for potential supportive living program participants. Collaborate with participants to develop and monitor progress on individualized Service Plans that are designed based on an assessment of needs and include goals and actions to be taken to achieve goals for both the qualifying individual and any family members with which he/she lives. Advocate for program participants Encourage development of social support, recreational activities, and involvement in neighborhood activities. Link participants with community, treatment, and recovery resources and services as appropriate and desired. Work cooperatively with treatment providers when participants choose to engage in treatment services. Obtain releases of information in accordance with existing laws and regulations and clearly inform the participant of his/her rights and agency policies. In collaboration with appropriate directors, design tools and collect data and qualitative information on participant progress and outcomes. When deemed appropriate, manage the process of transitioning participants from supportive housing to other housing, including providing assistance identifying appropriate housing, securing housing contracts, participant exit interviews, etc. Assist maintenance staff with monitoring and ensuring the quality and safety of the property. The case manager will be responsible for gathering pertinent information relevant to the client's evaluation, through the use of diagnostic tools, interviews, collateral contacts, urine screens and information from previous treatment providers. The case manager will be responsible for pertinent paperwork associated with the intake to include releases, fee contract, medical assessment, rights and responsibilities, case notes, diagnostic intake form, MSE, 5 axis, activities waiver, narrative summary, letters to referral source. • The case manager will be responsible for presenting the clients' evaluation to the treatment team during case conference, completing the case conference form and informing the client of the results of case conference, through phone contact or a face-to-face follow-up session. The case manager will schedule client with primary treatment counselor following client's acceptance of treatment recommendation. If client refuses treatment recommendation, client's record will be screened.QUALIFICATIONS
Bachelor's degree required in a relevant topic area. Minimum of 1 year of case management or similar experience required. 2 or more years and/or intensive case management experience preferred. CASAC preferred. Housing program experience preferred. Excellent communication skills. Ability to work a flexible schedule that may include weekdays, weekends, and evenings. Ability to work with individuals and families who have substance use disorder and a variety of life challenges that result in a need for support in order for the individual/family to remain stably housed. Knowledge of the varying modalities and philosophies of alcohol and substance use disorder treatment and recovery, including a solid understanding of and willingness to support harm reduction approaches. Commitment to frequent face-to-face meetings with participants and additional check-ins as needed via other communication modes (e.g. phone, etc.) Knowledge of community resources and programs, including but not limited to safety net programs. Ability to effectively communicate and collaborate with representatives from public and community agencies. Commitment to providing high-quality, complete participant notes. NYS driver's license and reliable method of transportation required. Pre employment background/post offer drug screening required. Must be free from active addiction and non program participant. This is an hourly nonexempt hourly position.Job Type:
