This is a fast-paced, field-based and office-integrated role requiring strong judgment, communication, and follow-through. This position is expected to run through November, with the potential to transition into a permanent role based on performance and business needs. Primary ResponsibilitiesClient Intake & Care Planning Conduct in-home consultations with prospective clients and family members Assess physical, cognitive, and psychosocial needs to develop individualized care plans Set clear expectations with families regarding scope of care, scheduling, and pricing Partner with internal team to ensure timely start of care Caregiver Matching & Onboarding to Case Identify and match appropriate caregivers based on client needs, personality, and logistics Facilitate caregiver/client meet & greets and ensure alignment before start of care Troubleshoot mismatches quickly and decisively Ongoing Care Coordination Serve as primary point of contact for assigned clients and caregivers Conduct regular follow-up with both clients and caregivers to ensure satisfaction and quality of care Proactively identify and resolve issues related to care delivery, scheduling, or caregiver performance Maintain accurate and timely documentation of all client interactions and updates Supervision & Quality Assurance Conduct supervisory visits in clients' homes to assess care quality and caregiver performance Provide coaching and feedback to caregivers in the field Escalate performance concerns and partner with leadership on resolution Team Communication & Operations Communicate all client needs, schedule changes, and updates clearly to the office team Collaborate closely with scheduling to ensure shifts are staffed appropriately Maintain strong internal alignment to support seamless operations Caregiver Recruitment & Hiring Support Participate in ongoing caregiver recruitment efforts Conduct phone screens and first-round interviews with caregiver candidates Evaluate candidates for both skill and cultural fit
