On My Watch case management's goal is to connect provider and community resources to enable clients to manage disease, remain in their communities, and prevent premature admissions to nursing facilities We're seeking a compassionate, organized Administrative/Program Assistant to support elderly individuals and persons with disabilities as they navigate resources and pursue eligibility for Medicaid Waiver services. This role is ideal for someone who is friendly, dependable, and committed to helping others with patience and respect. Key Responsibilities Conduct intake and screening calls for individuals seeking Medicaid Waiver services Collect and document essential information accurately Provide clear guidance on eligibility, documentation, and next steps Refer clients to community resources and support services Work independently from home while meeting deadlines and productivity expectations Qualifications Compassionate, patient, and service‑oriented Strong organizational skills and attention to detail Dependable and able to work independently Excellent communication skills Experience in the medical field or working with elderly individuals/persons with disabilities is a plus Join our team as a Program Assistant to play a vital role in supporting impactful programs through exceptional administrative excellence!
