Job Description

The Access Coordinator is a patient-facing care coordination role that supports safe, timely, and patient-centered transitions across the continuum of care. Working in close partnership with case managers, social workers, physicians, nurses, and the inpatient care team, the Access Coordinator engages directly with patients and families to facilitate patient throughput, discharge planning, transfers to higher or lower levels of care, and repatriation to home facilities. Ensure compliance with EMTALA, payer requirements, and hospital policies related to transfers, repatriation, and discharge planning. The Access Coordinator serves as a consistent point of contact for patients, families, and external partners throughout the transfer, repatriation, and discharge process. This role serves as a guide and advocate for patients and families during complex transitions, including repatriation of capitated members, coordination of post-discharge transportation, and securing timely follow-up care. The Access Coordinator ensures continuity of care by coordinating information exchange, follow-up appointments, and bedside support such as delivery of durable medical equipment, while maintaining strong relationships with external agencies, payers, and community providers. Basic technical and computer skills, minimum of 40 wpm Ability to multi-task and take direction from staff members belonging to different disciplines.
