Clinical Care Coordinator (LVN) Clinical Tustin, California Description Position at Tustin Healthcare Center Clinical Care Coordinator $75k-$80k DOE Full-time Tustin, CA Tustin Healthcare, a Generations Healthcare facility is in search of a Clinical Care Coordinator to join our team.
Job Description:
The primary purpose of the Clinical Care Coordinator is to conduct and coordinate the skilled services provided to the resident as it relates to assessments, documentation and the discharge process in accordance with the requirements of state and federal regulations and the policies, procedures and goals if the facility. Essential Duties (include but are not limited to):
  •  Evaluate and update patient health care plans
  •  Participate with the facility IDT to be sure the resident plan of care is maintained and followed
  •  Proficient in Medicare Rules, Regs and RAI Assessment skills
  •  Complete required skilled charting for residents following admission until the resident is discharged from skilled services
  •  Review medical forms, reports, evaluations, etc., as necessary for utilization, completeness and accuracy
  •  Act as a liaison with Case Managers
  •  Create plan of care for discharges
  •  Round with physicians when necessary
  •  Attend resident care conferences
Qualifications:
  • Must be a LVN with an active, unincumbered license to practice in CA
  • Must have, as a minimum, 2 years of experience in a skilled nursing facility.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
