POSITION DESCRIPTION TITLE
Care Coordinator
DEPARTMENT
Long Term Community Recovery Services
REPORTS TO
Director of Wellness and Long Term Community Recovery
FLSA STATUS
[Exempt/Non-Exempt]
PRIMARY FUNCTION
Serves as the primary point of contact for assigned long-term recovery cases; assisting clients in developing a recovery plan and in coordinating necessary services and resources to address the client's complex disaster recovery needs in order to re-establish normalcy.
KEY ROLES FOR
Care Coordinator (Essential Job Responsibilities):
  • Reaches out to clients assigned to them to schedule intake and assessment.
  • Performs assessment and verification of disaster recovery needs.
  • Works with clients to create a Recovery Plan that utilizes objectives and goals that are specific, measurable, achievable, realistic, and time sensitive
  • Monitors each clients' recovery progress and works with clients to revise and update their plan as the recovery progresses
  • Networks with other organizations to guide clients through sequence of delivery without duplication of benefits or services
  • Advocates with and for clients through activities including but not limited to: ○ preparing for and making case presentations on behalf of clients; ○ actively participating in long-term recovery groups; ○ providing support and advocacy with governmental and non-governmental agencies and organizations when necessary
  • Respects every client's right to privacy, protecting each client's confidential information
  • Works in a respectful, non-judgmental, and non-discriminatory manner
  • Insures that all necessary forms and releases are signed
  • Documents all conversations/touch-points with clients in SalesForce
  • Stays on top of community resources in collaboration with Care Coordination team and refers clients as needed
  • Completes closure with client including client satisfaction forms
RELATIONSHIPS
  • Works collaboratively with entire BGCM Long Term Recovery team
  • Collaboration with MCLTRG
  • Participates in supporting
DRC SKILLS/KNOWLEDGE REQUIRED
  • Strong interpersonal skills and ethical conduct
  • Excellent organizational skills
  • Excellent computer skills including word processing and spreadsheets
  • Empathy for people in disasters and connection to the community
  • Flexibility in changing circumstances and environments
  • Ability to work with people of all ages, ethnic and religious backgrounds, denominations and cultures
  • Is aware and respectful of each client's unique cultural and spiritual environment
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
  • Primarily office-based with some travel/field-based
  • Ability to sit or stand for extended periods and occasionally lift up to 25 lbs.
  • Work environment may include exposure to busy program activities and occasional outdoor events.

WORK ENVIRONMENT
Employees work primarily in a professional office setting with moderate noise levels. Occasional visits to program sites may expose employees to outdoor conditions and varied activity levels. Reasonable accommodations will be provided as needed.
