Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for patients recovering from stroke, brain, spinal cord, and other complex conditions. In this role, you'll lead discharge planning, manage utilization review, and collaborate with an interdisciplinary team to ensure safe, timely transitions of care. You will educate patients and families, navigate insurance and community resources, and support evidence-based, patient-centered rehabilitation plans that restore function, independence, and quality of life in a mission-driven setting. Responsibilities Coordinate individualized care plans for inpatient rehabilitation patients from admission through discharge Lead discharge planning to ensure safe, timely transitions to home or next level of care Conduct utilization review and manage authorizations with payers to support appropriate length of stay Collaborate with physicians, therapists, nurses, and social workers to optimize clinical and functional outcomes Educate patients and families on care plans, community resources, and post-discharge needs Assess psychosocial, financial, and support needs to connect patients with appropriate services Document case management activities in the EMR in compliance with regulatory and hospital standards Monitor outcomes and participate in quality improvement initiatives to enhance rehabilitation services Required Skills Case management Discharge planning Utilization review Interdisciplinary care coordination Electronic medical records (EMR) Insurance authorization and reimbursement Patient and family counseling Rehabilitation and post-acute care knowledge Care transition planning Outcome tracking and reporting
