Hours:
Monday-Friday, 8am- 4:30pm with Saturday coverage every 6 weeks.
What you will do:
Designs and develops consumer nutrition, health and wellness education programs and/or services targeting current user groups and data collection of risk indicators.This includes:
Identifies and develops evaluation criteria for assessing effectiveness of these programs. Performs nutritional, health and wellness screening and assessment. Manages ongoing nutritional care plans based upon the developmental needs of each patient-member. Prepares and completes patient-member records and documentation support according to licensure guidelines. Contributes to the nutrition-specific education/in-service training of patients, professional staff, colleagues, support staff and students when required. Provides appropriate documentation that summarizes the nutrition care plan in the patient's medical record, including nutritional assessment, diagnosis plan, implementation and progress toward needs. Demonstrates accountability for the proper use of patient's protected health information. Utilizes patient scheduling and monitors/maintains patient through electronic medical record.What you will need:
Education :
Master's Degree in Nutrition, required Completion of required nutrition and dietetic coursework through ACEND-Accredited didactic program or coordinated program in dietetics. Completion of an ACEND-accredited supervised practice through a dietetic internship, individualized supervised practice pathway or a coordinated program in nutrition and dieteticsLicense :
Current State of Illinois Dietitian Nutritionist licensure (LDN) within 90 of start date into position.Certification :
Registered Dietitian Nutritionist credential (RDN)Experience :
