Job Description
Job Summary We are seeking a dedicated and compassionate Community-Based Therapist to join our dynamic mental health team as we go into the community providing the service of Intensive In Home. In this role, you will provide comprehensive behavioral and mental health services within community settings, focusing on children, youth, and families. Your expertise will help clients navigate complex emotional, behavioral, and developmental challenges through evidence-based practices and culturally sensitive interventions. You will play a vital role in promoting mental wellness, supporting recovery, and fostering resilience in diverse populations. Responsibilities Conduct thorough mental health assessments, including diagnostic evaluations and client intake procedures, to identify individual needs and develop personalized treatment plans. Deliver evidence-based therapies such as cognitive behavioral therapy (CBT), trauma-specific interventions, play therapy, art therapy, and behavioral management techniques tailored to children and adolescents. Provide individual, group, and family counseling sessions to address issues related to mental health conditions, developmental disabilities, PTSD care, autism spectrum disorder (ASD), gender issues counseling, and more. Collect accurate clinical data through documentation of progress notes, medical records, treatment plans, and case management reports in compliance with HIPAA regulations and clinical ethical guidelines. Collaborate with multidisciplinary teams—including social workers, educators, medical professionals—and coordinate care to ensure holistic support for clients from diverse cultural backgrounds. Utilize motivational interviewing and crisis intervention strategies to support clients during acute episodes or emergencies effectively. Engage in ongoing professional development by staying current with evidence-based practices in mental health counseling, trauma-informed care, applied behavior analysis (ABA), and cultural diversity counseling. Experience Proven experience working with children and youth in community or clinical settings involving mental health assessments and behavioral therapy. Demonstrated expertise in home & community care environments with a strong understanding of social work principles. Hands-on experience with trauma-specific interventions, crisis management, conflict resolution, and gender issues counseling. Familiarity with medical documentation systems adhering to HIPAA standards and clinical ethical guidelines. Background working with individuals with developmental disabilities or autism spectrum disorder (ASD), including experience applying ABA techniques. Knowledge of medical terminology related to mental health conditions and experience working within multidisciplinary teams for case management. Ability to communicate effectively with clients from diverse cultural backgrounds while providing culturally sensitive counseling services.
