Job Description

As a Direct Support Professional at Sevita, you'll provide person-centered support to individuals with diverse abilities in home and community settings. You'll assist with daily living, health and safety, and community engagement while implementing individualized support plans. In this role, you'll promote independence, provide behavioral and emotional support, and maintain accurate documentation. Responsibilities Provide daily living and personal care support in home and community settings Implement individual support plans to promote independence and skills building Monitor, document, and report progress, behaviors, and health/safety concerns Support community participation, transportation, and recreational activities Collaborate with clinical and care teams to deliver person-centered services Follow behavior support plans and apply crisis de-escalation techniques Maintain a safe, clean environment and follow infection control standards Communicate effectively with individuals, families, and supervisors Assist with medication reminders and health-related tasks per policy Participate in ongoing training and professional development Required Skills Personal care assistance Behavioral support techniques Crisis de-escalation Medication assistance (per policy) Documentation and progress notes Infection control practices First aid and CPRPatient transfer and mobility support Basic computer skills/EMR use Community integration support
