Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; report findings accurately Support medication administration following prescribed guidelines and document all medical records in Electronic Health Record (EHR) systems Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments; ensure safety and comfort during daily routines Perform catheter care and assist with other medical procedures as trained; maintain strict adherence to home healthcare regulations and HIPAA confidentiality standards Facilitate patient transfers using appropriate methods such as Hoyer lifts or transfer belts; ensure safe mobility assistance at all times Support end-of-life care with compassion, dignity, and respect while maintaining patient comfort and emotional well-being Requirements Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice settings; assisted living or nursing home experience is highly valued Knowledge of medical terminology, medication management, and vital signs monitoring Certification in First Aid and Basic Life Support (BLS), including CPR; ability to respond swiftly in emergency situations Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient care Ability to lift heavy objects safely and perform physical tasks such as repositioning patients or performing heavy lifting when necessary Strong communication skills for effective client interaction and collaboration with healthcare teams Valid driver's license and reliable transportation preferred for home visits; adherence to home health care regulations and safety protocols Join us to be a vital part of a compassionate team that values your dedication.
