Licensed Vocational Nurse (LVN) - Residential Care Facility for the Elderly (RCFE) Raya's Paradise Residential Care Facilities is looking for an experienced and compassionate Licensed Vocational Nurse (LVN) to join our team. Position Overview The LVN is responsible for providing nursing oversight, resident care, staff support, medication management, documentation, and coordination of medical services. This position requires someone who is hands-on, dependable, organized, responsive, and comfortable working in a 24-hour residential care environment. The ideal candidate will be comfortable working with seniors, including residents with Alzheimer's disease and other dementia-related conditions , and will be able to communicate effectively with residents, families, caregivers, med techs, physicians, pharmacies, and management. Responsibilities Provide nursing care and supervision to residents, including vital signs, injections, skin checks, medication management, incontinence management, diabetes management, and monitoring of overall resident health. Monitor the safety and well-being of residents and follow up with facility staff to ensure resident care needs are consistently and accurately addressed. Review and update resident care needs and assessments as required. Assist with medication management, including medication storage, distribution, ordering, and staff training. Inspect MARs to ensure they are complete, accurate, and maintained appropriately for all residents. Coordinate with pharmacies and physicians regarding medication orders, refills, changes, discontinued medications, and other medication-related concerns. Follow up on physician orders and communicate with responsible parties, physicians, pharmacies, and administrators to ensure appropriate implementation and compliance. Participate in and assist with resident care plan meetings with families and responsible parties on an ongoing basis. Arrange and assist with medical and dental appointments and coordinate follow-up care as needed. Monitor residents for changes in physical, mental, and emotional condition and take appropriate action when concerns arise. Check residents for skin breakdown, injuries, changes in condition, or other health concerns and notify the appropriate medical provider and Administrator as needed. Provide first aid and CPR when necessary and within the scope of training and licensure. Notify the Administrator of resident incidents, significant changes in condition, injuries, and other reportable occurrences. Document assessments, incidents, care plans, and other required information accurately and timely in YARDI. Complete, review, and submit required incident and death reports to the California Department of Social Services as applicable. Maintain and supervise compliance with facility policies and procedures related to resident safety and care. Communicate effectively with caregivers, med techs, management, physicians, responsible parties, and other healthcare professionals. Provide guidance and support to care staff and help promote consistency and quality in resident care. Assist with ongoing observation and follow-up of residents as needed to ensure their health and safety. Maintain knowledge of and follow applicable California RCFE regulations, Title 22, licensing requirements, and facility policies and procedures. Schedule & Availability Requirements Because Raya's Paradise operates 24 hours a day, 7 days a week , flexibility and availability are important requirements for this position. Must be flexible with work hours and scheduling based on the operational needs of the facility. Must be open and willing to work weekends as needed . Must be willing to adjust your schedule when necessary to support resident care and the smooth operation of the facility. Must be comfortable answering work-related calls and responding to urgent or emergency nursing situations after regular working hours and on weekends when needed . Must be responsive to emergency calls received through the nurse extension and able to return calls and provide appropriate nursing direction and follow-up. Must be able to provide support and guidance to staff regarding resident care concerns, including after-hours situations when needed. This position requires a dependable LVN who understands that resident care needs may arise outside of a traditional Monday-Friday schedule. Qualifications & Requirements Graduate of an accredited nursing program. Current and active California LVN license in good standing. Minimum of 2 year of experience working in a skilled nursing, assisted living, RCFE, or similar residential care setting. Experience working with geriatric, dementia, Alzheimer's, or disoriented residents preferred. Current TB clearance. Current health screening/clearance as required. Current First Aid and CPR certification. Required fingerprint/background clearance through FBI and DOJ. Must be at least 18 years of age. Valid California driver's license, good driving record, and current automobile insurance. Ability to read, write, and communicate effectively in English. Strong written, verbal, and listening communication skills. Excellent organization, time-management, and follow-through skills. Strong attention to detail and ability to accurately complete clinical and regulatory documentation. Ability to communicate professionally and empathetically with residents, families, responsible parties, and coworkers. Demonstrated compassion and patience when working with the senior population. Ability and willingness to work with residents with Alzheimer's disease and other dementia-related disorders. Knowledge of, or willingness to learn, California RCFE licensing requirements and Title 22 regulations . Knowledge of medication management, resident care planning, documentation, and regulatory compliance. Ability to work independently while also collaborating effectively with the management and care team. Must demonstrate professionalism, reliability, accountability, and sound judgment.
