Job Summary Harmoni Care Solutions is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide In-Home Nursing Respite (IHNR) services to a client in Greenville, MS. The client is bed-bound and requires total assistance with all activities of daily living. The client receives nutrition through a G-tube and has a colostomy, and a mechanical lift is available in the home. This is a part-time position, Monday-Wednesday, 7:00am-5:00pm (30 hours per week), providing skilled nursing support and personal care in the client's home. Responsibilities Provide total assistance with bed baths, personal hygiene, oral care, grooming, and dressing Perform safe transfers using the in-home mechanical lift, following proper technique and body mechanics Turn and reposition the client per care plan, and monitor skin integrity to prevent pressure injuries Administer G-tube feedings, water flushes, and medications through the G-tube per physician orders Provide G-tube site care, including inspecting for redness, leakage, irritation, or signs of infection Recognize and promptly report G-tube complications, such as dislodgement, blockage, leakage, or feeding intolerance, to the supervising RN Provide colostomy care, including emptying and changing the pouch and appliance, and caring for the stoma and surrounding skin Monitor and document colostomy output and stoma appearance, and report abnormalities to the supervising RN Administer medications per physician orders and care plan Monitor and document the client's health status, vital signs, and any changes in condition Report changes in the client's condition to the supervising RN and/or care team promptly Maintain accurate and timely documentation of services provided Follow the individual's approved care plan and Harmoni Care Solutions' policies and procedures Maintain a safe, clean, and supportive home care environment, following infection control practices Communicate professionally with the client, family members, and care team Qualifications Current, unencumbered Mississippi LPN license (or multistate compact license) Current CPR certification Experience with total-care, bed-bound clients, including cerebral palsy, G-tube feeding, and ostomy care, preferred Ability to safely operate a mechanical patient lift Clear background check Reliable transportation Schedule Monday-Wednesday, 7:00am-5:00pm (30 hours per week)
