7:00pm-7:00Am (Night) Duties Provide comprehensive nursing care to patients in multiple healthcare settings, including long-term care facilities, hospitals, assisted living communities, and home care environments. Conduct patient assessments, vital signs monitoring, and physical examinations to evaluate health status accurately. Administer medications, injections, IV infusions, and perform catheterization following strict protocols and safety guidelines. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage patient documentation using electronic health record (EHR) systems like Epic or Cerner, ensuring accuracy and confidentiality in compliance with HIPAA regulations. Support emergency response efforts by managing airway management, first aid, CPR, and acute pain management techniques. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Educate patients and caregivers on health management strategies including nutrition, medication adherence, and post-discharge care planning. Ensure infection control practices are rigorously followed to maintain a safe environment for patients and staff. Participate in case management activities to coordinate long-term care services and discharge planning. Qualifications Valid Licensed Practical Nurse (LPN) license in good standing; current state licensure required. Proven experience working in diverse clinical settings such as critical care units (ICU), emergency rooms, medical-surgical units, or long-term care facilities. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with medical documentation is essential. Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures such as venipuncture, tube feeding, sterile technique, and catheter care. Experience working with geriatric populations or individuals with disabilities; knowledge of dementia and Alzheimer's care is highly desirable. Ability to perform triage assessments efficiently in urgent or emergent situations; experience in emergency medicine or Level I trauma centers is a plus. Competency in infection control standards and acute pain management techniques. Excellent communication skills for effective caregiver collaboration and patient education. Certification in Basic Life Support (BLS) required; additional certifications such as IV insertion or advanced cardiac life support (ACLS) are advantageous.
