Opportunities available in the heart of Dallas The Community Health Nurse (LVN) provides nursing support for HHM Health's pediatric mobile clinic, community outreach initiatives, population health screening programs, and employee health services. The position delivers preventive nursing services, health screenings, patient education, immunizations within scope, and referral coordination while supporting community engagement and employee wellness. The Community Health Nurse serves as a clinical resource during outreach events and promotes safe, high-quality care in community and nontraditional healthcare settings.

Here's a sneak peek at what you'll do : Support pediatric providers during mobile clinic events, including patient intake, immunizations, medication administration within LVN scope, specimen collection, point-of-care testing, and patient education. Provide nursing support at community outreach events serving adults and children. Perform community health screenings including blood pressure, blood glucose/A1C, HIV testing, vision screening, BMI, and other approved screenings. Identify abnormal findings and promptly escalate them to the appropriate provider. Educate patients on chronic disease prevention, nutrition, immunization, preventive care, and healthy lifestyles. Coordinate referrals to HHM Health providers and community resources. Assist with employee health activities including influenza campaigns, TB screening, immunization review, wellness initiatives, and occupational health support. Maintain mobile clinic readiness, supplies, medications, vaccines, emergency equipment, and infection prevention standards. Administer pediatric and adult vaccines as required Conduct POC testing as required during outreach events. Document services accurately in organizational systems. Participate in quality improvement, outreach planning, and community partnership activities. Ensure compliance with nursing standards, HIPAA, OSHA, HRSA, and organizational policies. Travel to community sites and perform other duties as assigned. What you need to succeed To be a productive member of our team, you will have a pleasant and professional demeanor, be a self-starter, the ability to work independently, strong communication skills, and the ability to preserve confidentiality. You will also have the following: Current unrestricted Texas LVN license in good standing. Pediatric and adult medicine nursing experience Current BLS certification. Minimum two years of clinical nursing experience preferred. Experience in pediatrics, community health, public health, primary care, or mobile health preferred. Valid Texas driver's license and ability to travel throughout the community.
