ASAP Start Date Day Shift 4x10, with Rotating Weekends and Holiday Must be 50 miles from facility Free Standing facility 5 wings, 2 assisted living 107 Beds Responsibilities Provide direct patient care, including administering medications and treatments as prescribed. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Document patient information accurately in electronic health records systems. Collaborate with registered nurses and other healthcare professionals to develop and implement care plans. Educate patients and families on health management and disease prevention. Maintain a safe and clean environment for patients by adhering to infection control protocols. Qualifications Valid Wisconsin LPN license 1 year of LTC experience within the last 2 years. Strong patient assessment skills with the ability to recognize changes in patient conditions. Excellent communication skills with the ability to work collaboratively within a healthcare team. Commitment to maintaining HIPAA compliance and ensuring patient confidentiality.
