Description We are looking for a highly organized and detail-oriented Health Information Clerk to join our Health Information Management team at our hospital in Chandler, AZ. The Health Information Clerk will support the accurate, timely, and confidential management of patient health information while assisting with the day-to-day operations of the HIM department.

Responsibilities:

+ Retrieved and prepared patient medical records for authorized hospital departments and healthcare providers. + Scanned and indexed medical records and documents into the electronic health record system. + Verified that documents were accurately associated with the correct patient record. + Filed, organized, and maintained paper and electronic health information according to department procedures. + Assisted with the tracking, retrieval, and reconciliation of incomplete or missing medical records. + Processed routine health information requests according to established hospital procedures and privacy requirements. + Entered and updated patient and medical record information accurately in designated systems. + Assisted with maintaining the integrity, completeness, and accessibility of patient health records. + Responded to routine inquiries from patients, hospital staff, and other authorized individuals. + Followed HIPAA regulations, hospital privacy policies, and procedures for safeguarding confidential patient information. + Identified and reported discrepancies, missing documentation, or potential record integrity issues to appropriate HIM staff. + Assisted with quality control activities to ensure medical records were complete, accurate, and properly organized. + Maintained productivity and accuracy standards established by the HIM department. Requirements + Previous hospital Health Information Management experience. + Previous experience working with an electronic health record system. + Knowledge of medical record filing, scanning, indexing, and document management processes. + Familiarity with HIPAA regulations and healthcare privacy practices. + Knowledge of common medical terminology, abbreviations, and healthcare documentation. + Previous experience with Release of Information or medical-record requests. + Experience working with high-volume records or document-processing environments. + Familiarity with HIM policies, procedures, and record-retention requirements. + Experience with quality assurance or medical-record auditing processes. + Ability to prioritize multiple tasks while maintaining accuracy and productivity.
