60% of monthly collections Overview We are seeking a compassionate and motivated Nurse Practitioner (FNP or AGNP) to join our post-acute care team. This role focuses on delivering high-quality patient care in skilled nursing facilities and supporting the recovery and management of medically complex patients. The ideal candidate will demonstrate strong clinical acumen, excellent communication skills, and a collaborative mindset to ensure the best outcomes for our patients. Responsibilities Perform comprehensive health assessments and physical exams for nursing home patients. Develop and implement individualized treatment and care plans. Diagnose and manage acute and chronic conditions in the post-acute setting. Educate patients and responsible parties on health maintenance, disease prevention, and medications. Accurately document visits and maintain compliance with HIPAA and clinical guidelines. Collaborate with physicians, facility staff, and interdisciplinary teams to coordinate care. Participate in quarterly provider meetings and take weekday call (Monday-Friday, 7 AM-7 PM), as needed. Qualifications Board Certified Nurse Practitioner (FNP or AGNP) 1-2 years of clinical experience preferred Skilled Nursing Facility (SNF) RN experience is a plus Must be able to round on patients in Seguin, TX , twice a week Key Skills Strong knowledge of medical coding and terminology Proficient in interpreting lab results and writing clinical orders Comfortable with durable medical equipment (DME) and post-acute care needs Excellent interpersonal and patient communication skills Familiarity with urgent care protocols Team-oriented with the ability to work independently when neededCompensation & Benefits Collections:
