Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, managing medication administration, and reporting changes to healthcare professionals. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments, ensuring their safety and comfort through memory care techniques. Support clients with medical procedures such as catheter care, and assisting with medical documentation while maintaining strict adherence to HIPAA and home healthcare regulations. Facilitate client communication by engaging in meaningful interaction and ensuring their emotional well-being during visits. Maintain accurate medical records and documentation using Electronic Health Record (EHR) systems or Electronic Medical Record (EMR) systems. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment. Skills Proven experience working with geriatric patients or individuals with disabilities in home healthcare or long-term care settings. Strong knowledge of medical terminology, patient care procedures, and home health care assistance protocols. Ability to perform patient observation accurately and respond swiftly to changing conditions. Familiarity with home health agency operations, hospice care, and end-of-life support practices. Excellent communication skills for client interaction and coordination with healthcare teams. Competence in using EMR/EHR systems for medical documentation and record keeping. Certification in First Aid, Basic Life Support (BLS), and CPR is required; additional certifications such as hospice care or dementia training are a plus. Level 2, Physical and TB. Physical ability to perform lifting safely and assist with patient transfers using proper techniques like the Hoyer lift or patient transfer methods. Knowledge of home & community care regulations ensures compliance with legal standards governing home health services.
