Job Summary We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our dynamic healthcare team. In this vital role, you will provide exceptional patient care, support clinical staff, and ensure the comfort and safety of our patients in their homes. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and dignity. Monitor vital signs including blood pressure, pulse, temperature, and respiration, accurately documenting all findings in electronic health records (EHR) systems. Maintain patient confidentiality in accordance with HIPAA regulations while managing sensitive medical records and documentation. Use safe manual handling practices and equipment like Hoyer lifts to transfer patients safely, reducing risk of injury. Observe and report changes in patient condition or behavior promptly to the nursing staff, especially for individuals with dementia or behavioral health needs. Collaborate effectively with interdisciplinary teams in settings such as long-term care facilities, hospitals, assisted living communities, or home care environments to deliver comprehensive patient-centered care. Qualifications Valid certification as a Nursing Assistant (CNA) from an accredited program. Strong communication skills to effectively interact with patients of all ages and coordinate with healthcare teams. Ability to perform heavy lifting and manual handling safely while adhering to safe lifting practices. Prior experience in home health and hospice is a plus, but not a requirement.
