Supportive, team-oriented environment Opportunity to build relationships with residents and families Consistent schedule with a mix of clinical and administrative work Growing community with strong leadership support What You'll Do Lead the Admissions Process Conduct pre-admission and move-in assessments Collaborate with Sales & Marketing to review incoming resident information Partner with the Director of Nursing to determine appropriate admissions Ensure Accurate Clinical Assessments Evaluate residents' physical, cognitive, and functional status Determine appropriate levels of care and service needs Monitor changes in condition and complete reassessments as needed Develop & Maintain Care Plans Complete and update General Service Evaluations (GSE) and Service Plans (GSP) Initiate Health Care Plans (HCP) for residents receiving additional services Communicate care updates clearly with residents, families, and the care team Collaborate Across Teams Work closely with nursing leadership, administration, and operations Participate in Level of Care meetings and care planning discussions Ensure smooth, supportive transitions for new residents What We're Looking For Active LPN or RN license (Michigan, in good standing) Experience in assisted living, senior care, or clinical assessments preferred Strong clinical judgment and attention to detail Excellent communication and interpersonal skills Ability to work both independently and as part of a team What Makes You a Great Fit You're compassionate but also highly organized You're confident making clinical decisions You enjoy working with seniors and supporting families You thrive in a role that blends clinical care with coordination Join Our Team If you're passionate about senior care and want to play a key role in welcoming new residents into a supportive community, we'd love to hear from you.
