Responsibilities Assist patients and residents with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor vital signs, record medical information accurately, and report changes to healthcare professionals promptly. Support medication administration under supervision, ensuring proper documentation in electronic health records (EHR) systems while maintaining patient confidentiality in compliance with HIPAA regulations. Provide specialized care for individuals with conditions such as dementia, Alzheimer's disease, developmental disabilities, or behavioral health needs. Perform patient transfers using safe lifting practices and appropriate transfer methods to ensure safety for both the patient and caregiver. Maintain aseptic techniques during procedures like catheter care or wound management to prevent infections. Document all care activities thoroughly within medical documentation systems such as EMR/EHR platforms, ensuring accuracy and completeness of records. Skills Proven experience working with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities. Knowledge of medical terminology, anatomy, physiology, and basic life support (CPR) certification. Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for efficient medical record management. Ability to perform heavy lifting safely and execute manual handling techniques effectively. Strong communication skills to collaborate with healthcare teams and provide compassionate patient service. Experience working in various healthcare environments including hospitals (Level I & II Trauma Centers), long-term care facilities, assisted living communities, or home care settings. Understanding of infection control practices and aseptic techniques essential for safe patient care.
