Duties Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems. Administer medications via injections, IV infusions, catheterizations, and other methods following strict safety protocols and HIPAA confidentiality policies. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, memory care, or acute pain management. Assist with diagnostic evaluations including specimen collection/processing and medical imaging procedures. Manage airway maintenance, ventilator support, tube feeding, and sterile techniques to ensure optimal patient safety. Collaborate with interdisciplinary teams on discharge planning, case management, and treatment adjustments based on patient condition and triage assessments. Document all patient interactions accurately in EMR/EHR systems while maintaining clinical confidentiality at all times. Support patients across various settings including skilled nursing facilities, hospitals (Level I & II Trauma Centers), emergency rooms, outpatient clinics, and home & community care environments. Engage with individuals with disabilities or developmental challenges through tailored care plans that promote independence and dignity. Demonstrate proficiency in advanced nursing procedures such as cardiac catheterization prep, spinal taps, suturing, or neonatal ventilator management when applicable. Experience Valid

RN/LPN/CNA

license with current certification to practice in your state. Prior experience working in hospital medicine, ICU (Intensive Care Unit), emergency medicine or trauma centers is highly desirable. Proven background in long-term care facilities or skilled nursing settings with familiarity in geriatric and dementia care. Experience working with EMR/EHR systems such as Epic or Cerner to ensure accurate medical documentation and record management. Knowledge of ICD coding (ICD-9 & ICD-10), CPT coding for procedures, and familiarity with medical terminology is preferred. Hands-on experience with IV insertion/infusion therapy, catheter care, phlebotomy, and medication administration techniques. Background in acute care environments including urgent care or post-acute settings demonstrates adaptability to fast-paced situations. Skilled at patient assessment and triage processes to prioritize urgent needs efficiently while maintaining high standards of safety and compassion.
