- FULL-TIME EVENINGSSummaryProvides hands on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist.
Job DutiesProvides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.
Minimum QualificationsHigh School Diploma/GED1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
Knowledge of medical terminology.
Strong time management skills.
Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
Ability to work in patient homes.
Ability to work with and support terminally ill patients and their family members/caregivers.
