Job Purpose:

The Enhanced Care Manager (ECM) provides comprehensive, non-clinical care coordination and navigation support for high-risk, vulnerable members within a community clinic setting. Operating under California's CalAIM initiative , this position manages a diverse caseload to address complex medical, behavioral health, and social determinants of health (SDOH).You will coordinate timely community resources, manage care transitions, and collaborate effectively with the multidisciplinary Harmony Health team to improve member health outcomes.

Essential Duties:

Preferred Experience:

Medical Assistant (MA), or other healthcare-related background.

Core Experience:

Prior experience in care coordination, case management, social work, or community health environments preferred.

Industry Knowledge:

Familiarity with Medi-Cal, Partnership HealthPlan of California (PHC), CalAIM, or specialized ECM programs highly preferred.

Systems:

Experience operating Electronic Health Records ( Epic proficiency is highly preferred ) and tracking systems.

Skills:

Exceptional organizational habits, clear verbal and written communication capabilities, and strong narrative documentation skills.

Demonstrated ability and deep empathy to work effectively with high-risk, unhoused, and vulnerable populations.

Working Conditions & Physical Demands:

Work is split between a stable healthcare clinic office setting and frequent local travel for community outreach, home visits, or client meetings at external resource locations (shelters, community centers). Must be able to use a computer and keyboard for extended periods. Requires manual dexterity to operate basic clinical tools (e.g., automated blood pressure cuffs). Ability to walk moderate distances, including navigating hills, uneven surfaces, and varying terrain during outreach. Occasional sitting, standing, bending, kneeling, and light to moderate lifting (up to 20 lbs). Ability to speak and write clearly in the English language.
