About the Role We're seeking an experienced Clinical RN Manager to provide clinical leadership and oversight for our Home Health team. This position is responsible for supervising field clinicians, ensuring compliance with Medicare Conditions of Participation and CHAP standards, supporting quality outcomes, and partnering with agency leadership to deliver exceptional patient care. This role blends leadership with clinical practice, allowing you to remain connected to patient care while supporting and developing an outstanding team of clinicians. What You'll Do Supervise and mentor a team of home health clinicians, providing ongoing clinical guidance, coaching, and support. Review and approve OASIS assessments, plans of care, physician orders, and clinical documentation for accuracy, compliance, and quality. Ensure compliance with Medicare Conditions of Participation, state regulations, and CHAP accreditation standards. Monitor patient outcomes and quality metrics, implementing performance improvement initiatives as needed. Participate in case conferences and interdisciplinary team meetings to ensure coordinated, patient-centered care. Support clinicians with complex patient cases, clinical decision-making, and problem resolution. Conduct supervisory visits, clinical competency evaluations, and annual performance reviews. Assist with orientation, onboarding, and ongoing education for new and existing clinical staff. Collaborate with intake, scheduling, therapy, and administrative teams to ensure efficient coordination of care. Provide direct patient care and perform skilled nursing visits as needed to support patient care and operational needs. Partner with agency leadership to foster a positive, collaborative, and patient-focused culture. What We're Looking For Current Registered Nurse (RN) license in New Hampshire, in good standing. Minimum of three years of recent home health experience, including strong OASIS knowledge. Previous leadership or supervisory experience in home health preferred. Thorough understanding of Medicare regulations, Conditions of Participation, and home health documentation requirements. Strong clinical judgment, communication, organization, and leadership skills. Experience with electronic medical records (MatrixCare experience is a plus). Current CPR certification. Valid driver's license, reliable transportation, and ability to travel throughout the service area.
