Position Summary We are seeking a compassionate and experienced Registered Nurse (RN) / Care Manager/NURSE to join our office-based primary care team. The ideal candidate will have experience in clinical care coordination, patient education, and managing both acute and chronic health conditions in an outpatient setting.
