Job Summary We are seeking a dedicated and energetic Occupational Therapist (OT) to join our dynamic healthcare team. In this role, you will provide expert occupational therapy services to geriatric patients in our skilled nursing facilities. Your primary focus will be on enhancing functional independence, supporting recovery, and improving quality of life through evidence-based interventions. You will collaborate with multidisciplinary teams to develop personalized care plans that address each patient's unique needs, including those with neurodevelopmental disorders, disabilities, or recovering from injury or illness. Duties Conduct comprehensive occupational therapy assessments to evaluate patients' functional abilities and needs Develop and implement individualized treatment plans aimed at improving daily living skills, motor functions, and cognitive abilities Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and hygiene to promote independence Support discharge planning by coordinating care transitions and providing home or community-based therapy services when appropriate Collaborate with speech therapists, physical therapists, physicians and case managers to ensure holistic patient care Document clinical progress accurately using electronic health records (EHR) management systems while maintaining detailed medical documentation and adhering to medical terminology standards Provide specialized interventions for geriatric populations. Educate patients and their families on therapy goals, strategies for ongoing support, and adaptations for home or school environments Participate in clinical interprofessional collaboration to optimize treatment outcomes in skilled nursing facilities. Qualifications Valid license as an Occupational Therapist (OT) in the relevant jurisdiction and has passed the NBCOT Exam. Proven experience working with individuals with disabilities with geriatric patients in skilled nursing facilities. Strong knowledge of anatomy, physiology, medical terminology, and evidence-based practice principles in occupational therapy Experience working with clients on treatment plan development for conditions such as neurodevelopmental disorders, post-injury rehabilitation, or chronic disabilities Skilled in implementing occupational therapy programs tailored to client needs within skilled nursing facilities. Ability to communicate effectively with geriatric clients. Proficiency in medical documentation standards and electronic health records (EHR) management systems Knowledge of assistive devices and adaptive equipment use for enhancing client independence in ADLs and mobility tasks Demonstrated ability to work collaboratively within multidisciplinary teams in fast-paced healthcare settings such as skilled nursing facilities or post-acute care environments Join us if you're passionate about empowering individuals through compassionate occupational therapy!
