Job Description

Job Overview We are seeking a dedicated and energetic Physical Therapist (PT) to join our dynamic healthcare team. In this vital role, you will deliver exceptional patient care by developing personalized treatment plans, conducting thorough assessments, and facilitating rehabilitation across diverse settings. Your expertise will help improve mobility, reduce pain, and promote overall well-being for patients of all ages. Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to identify treatment needs. Develop and implement tailored care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and patient education. Manage patient rehabilitation programs in our skilled nursing facility. Document clinical findings accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions between care settings. Conduct treatment sessions focused on geriatrics, and other specialized areas to optimize recovery outcomes. Engage in ongoing patient communication to motivate adherence to therapy regimens and address concerns effectively. Qualifications Valid state licensure as a licensed Physical Therapist (PT) along with the NPTE Exam. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Demonstrated ability to develop effective treatment plans tailored to individual patient needs. Experience working with geriatric patients, pediatric populations, or neurological conditions is highly desirable. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination. Excellent client communication skills coupled with the ability to collaborate effectively with healthcare teams. Pay:
$50.00 - $60.00 per hour Expected hours: 2.0 - 20.0 per week
