Category Pharmacy - Pharmacist Job Type Contract A PRN Pharmacist opportunity is available to provide critical coverage in a 34-bed long-term acute care hospital (LTACH) specializing in critical illness recovery. This role supports medication distribution, sterile product preparation, and comprehensive clinical pharmacy services to ensure optimal patient outcomes. The position emphasizes medication reconciliation, discharge counseling, therapy monitoring, and adherence to regulatory standards.

Contract, PRN/As-Needed Daytime Coverage Key Responsibilities:
Conduct thorough medication reconciliation and develop accurate home medication lists prior to admission. Provide patient and family discharge medication counseling. Monitor drug therapies for contraindications, interactions, allergies, and appropriateness. Offer clinical consultation and medication recommendations to medical staff. Participate actively in antimicrobial stewardship, including interpretation of culture results and infection management. Perform pharmacokinetic and anticoagulation dosing and monitoring. Support medication-use review, quality assurance, and drug utilization initiatives. Maintain medication inventory and oversee procurement processes. Prepare, verify, and dispense sterile and non-sterile medications per standards. Manage emergency meds, controlled substances, and conduct regular compliance reviews. Educate pharmacy personnel and other healthcare professionals, promoting interdisciplinary collaboration.
Qualifications and Desired Experience:
Active pharmacist license applicable to the assignment. Hospital/inpatient pharmacy experience preferred, with LTACH familiarity beneficial. Proficiency in EPIC clinical systems. Strong clinical knowledge combined with excellent communication skills. Ability to work independently in an LTACH environment. Experience with medication reconciliation, antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation monitoring. Availability for PRN/as-needed daytime shifts.
