In this role, you will provide expert physical therapy services to diverse patient populations, including geriatric, pediatric, neurological, orthopedic, and inpatient clients. Your expertise will help improve mobility, reduce pain, and promote overall well-being through personalized treatment plans. The ideal candidate is energetic, detail-oriented, and passionate about delivering exceptional patient care. Responsibilities Develop and implement individualized treatment plans based on thorough assessments of patients' needs and goals. Conduct patient evaluations, including physiology and anatomy assessments, to determine appropriate therapy interventions. Deliver hands-on therapy techniques such as trigger point therapy and other physiotherapy modalities to facilitate recovery. Coordinate discharge planning and collaborate with healthcare professionals across inpatient, outpatient, home health, and community care settings. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR/EMR systems). Monitor patient progress regularly, adjusting care plans as needed to optimize outcomes. Educate patients on exercises, mobility techniques, and injury prevention strategies to support ongoing rehabilitation. Experience Proven experience practicing in a physical therapy role within outpatient clinics. Demonstrated expertise in outpatient rehabilitation center environments with a focus on orthopedics. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Experience developing comprehensive care plans tailored to each patient'sneeds. Familiarity with outpatient experience including outpatient clinic operations and outpatient rehabilitation programs. Proficiency managing electronic health records (EHR/EMR systems) for accurate patient documentation. Ability to collaborate effectively with multidisciplinary healthcare teams to ensure seamless patient care transitions.
