Duties Conduct comprehensive assessments of patients' physical abilities, mobility, and functional status in their home environment. Develop individualized care plans tailored to each patient's needs, goals, and medical conditions. Implement evidence-based physiotherapy techniques such as trigger point therapy, manual therapy, and therapeutic exercises to promote healing and mobility. Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care. Manage discharge planning by preparing detailed documentation and ensuring smooth transitions to outpatient or community services. Maintain accurate clinical documentation adhering to healthcare standards and electronic health records (EHR) management protocols. Educate patients and their families on treatment routines, safety measures, and home exercise programs to support ongoing recovery. Skills Proven experience practicing in a physical therapy role within home health or outpatient rehabilitation settings. Strong knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to inform treatment strategies. Familiarity with medical terminology and clinical documentation standards for accurate record keeping. Ability to develop effective treatment plans that incorporate discharge planning and patient rehabilitation goals. Experience working with diverse populations including pediatric, geriatric, inpatient hospital, outpatient clinic, and community care environments. Proficiency in electronic health records (EHR) systems or EMR systems for efficient documentation and communication. Excellent client communication skills to motivate patients and ensure understanding of care instructions. Knowledge of outpatient clinic experience or inpatient hospital experience is highly desirable for comprehensive patient support.
