Responsibilities include:
Assessing patients' physical conditions and developing personalized treatment plans. Implementing evidence-based therapies to promote mobility, independence, and wellness. Working directly with patients in their homes, primarily 1-2 days a week. Managing Part B-only documentation, resulting in less charting and more patient interaction.
Qualifications & Skills:
Passion for therapy and helping patients achieve their full potential. Strong assessment and treatment plan development skills. Ability to work independently in a home health setting.
