  • 50/hour Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team.
This position offers the opportunity to work with residents in both short-term rehabilitation and long-term care while helping them improve mobility, strength, balance, and overall function.
Position Responsibilities:
Perform physical therapy evaluations and establish individualized treatment plans Provide treatment focused on mobility, gait training, transfers, balance, strengthening, endurance, and pain management Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members Monitor resident progress and adjust treatment plans as appropriate Educate residents and families on exercises, mobility techniques, and safety recommendations Maintain accurate and timely documentation Why Join Civita Care Center at

Qualifications:
