This role offers the opportunity to help residents regain mobility, improve function, and achieve their rehabilitation goals in a supportive skilled nursing environment.
Position Responsibilities:
Deliver physical therapy treatments under the supervision of the Physical Therapist Assist residents with therapeutic exercises, gait training, balance activities, transfer training, and mobility interventions Support individualized treatment plans focused on improving safety and functional independence Monitor resident participation and communicate progress to the supervising PT Collaborate with nursing and interdisciplinary team members to support resident care Complete accurate and timely documentation Why Join Civita Care Center at

Qualifications:
