We provide skilled home health services to patients in the comfort of their homes. We are looking for PTs who are committed to helping patients improve mobility, strength, balance, safety, and independence.

RESPONSIBILITIES
Evaluate patients and develop individualized physical therapy treatment plans. Provide physical therapy services according to the plan of care. Assess mobility, strength, balance, coordination, gait, and functional abilities. Provide therapeutic exercises and functional training. Assist patients in improving mobility, independence, and safety in the home. Evaluate the patient's home environment and identify potential safety concerns. Educate patients and caregivers regarding exercises, mobility, transfers, fall prevention, and safe use of assistive devices. Monitor patient progress and modify treatment as clinically appropriate. Communicate patient progress and changes in condition with the physician and interdisciplinary team. Complete accurate and timely clinical documentation. Participate in care coordination and interdisciplinary communication. Follow Medicare, Florida, agency, infection prevention, and patient safety requirements. Maintain patient confidentiality and comply with HIPAA requirements.
QUALIFICATIONS
Current and unrestricted Florida Physical Therapist (PT) license . Current BLS/CPR certification , if required by agency policy and applicable position requirements. Strong clinical assessment and communication skills. Ability to work independently in a home health setting. Reliable transportation and ability to travel to patient homes throughout Broward County. Ability to complete required documentation accurately and on time. Home health experience is preferred but not required.

Original professional documents do not need to be submitted unless Zenith Care Home Health specifically requests an original for verification.
