Job Description
Jackson Therapy Partners is seeking a full-time Physical Therapist to deliver high-quality, patient-centered care across a variety of clinical settings. In this role, you will evaluate patients, develop and implement individualized treatment plans, and document progress using EMR systems. You'll collaborate closely with physicians, nurses, and rehab teams while educating patients and families on exercises and self-management. Responsibilities Evaluate patients and develop individualized physical therapy treatment plans Deliver therapeutic interventions including exercise, manual therapy, and modalities Document assessments, goals, and progress accurately in EMR systems Collaborate with physicians, nurses, and interdisciplinary rehab teams Educate patients and families on home exercise programs and self-management Adjust care plans based on patient response and functional outcomes Ensure compliance with state practice acts, payer rules, and documentation standards Contribute to discharge planning and safe transition of care Maintain professional licensure, certifications, and continuing education Uphold clinician-first, patient-focused values and represent Jackson Therapy Partners professionally at client sites Required Skills Orthopedic assessment and treatment Neurologic rehabilitation Post-surgical rehab and discharge planning Gait and balance training Therapeutic exercise and manual therapy Pain management modalities Care plan development and documentation (EMR) Interdisciplinary care coordination Patient and family education Outcome measurement and quality reporting
