Benefits Key Requirements:

Active NJ Physical Therapist license or valid compact privilege DPT or equivalent entry-level PT degree Valid driver's license, reliable transportation, and current auto insurance Experience providing physical therapy independently in community-based settings Strong clinical documentation and communication skills Comfortable treating adults and older adults Experience with home/community-based, assisted living, or group-home settings is relevant Ability to work with DDD/developmental disability, Medicare Part B, commercial insurance, and private-pay patients

Role Focus:

One-on-one outpatient PT services in patients' homes, assisted living facilities, and group homes Evaluations, treatment planning, mobility/balance/strength training, transfers and patient safety Caseload management and clinical documentation Coordination with families, physicians, PTAs, and other therapy team members

Important:

This is community-based outpatient therapy , not a traditional home health position.
