1 Description American Traveler is seeking a Physical Therapist for a home health/hospice setting requiring 2+ years of experience and OASIS assessment proficiency. Details Home health/hospice setting providing skilled physical therapy services to patients in their homes Schedule is Monday through Friday, 8am to 4pm, with 2-3 weekend days required Uses McKesson for clinical documentation and visit management Coordinates care with physicians, nurses, caregivers, PTAs, PT students, and other rehab team members Requirements Minimum 2 years of physical therapy experience required State license required upon acceptance (license required if applicable) Experience with OASIS assessments and home health documentation preferred Knowledge of APTA guidelines and federal/state home health regulations Ability to supervise and direct PTAs, PT students, and home care aides Skilled in performing patient health assessments such as PT/INR and blood pressure monitoring Skilled in performing procedures such as staple/suture removal and wound care Additional Information Responsibilities include comprehensive OASIS and PT assessments, plan of care development, and skilled treatment delivery Acts as Care Manager coordinating patient care across the interdisciplinary team Completes required documentation to support billing, insurance coverage, and regulatory compliance Participates in in-service training and community outreach for rehab services Coordinates safe patient discharges and transfers in accordance with policy Participates in peer review audits and quality improvement initiatives Open to first-time travelers
