Primary Care Physician

Job Description

Position Overview An established outpatient primary care practice in Jupiter, Florida is seeking a Primary Care Physician to join its growing team. This position is designed for a physician who enjoys developing long-term relationships with patients and practicing within a value-based care model. The focus is on proactive, preventive care, chronic disease management, care coordination, and improving overall patient outcomes. The physician will work alongside a collaborative clinical team with the resources and support needed to provide comprehensive, patient-centered primary care. Responsibilities Provide comprehensive primary care to an established adult patient population Diagnose, treat, and manage acute and chronic medical conditions Develop individualized treatment and preventive care plans Manage chronic conditions such as diabetes, hypertension, cardiovascular disease, and other common primary care conditions Conduct routine examinations, screenings, and preventive health visits Coordinate care with specialists, hospitals, and other healthcare professionals when appropriate Review laboratory results, diagnostic studies, medications, and treatment plans Maintain accurate and timely clinical documentation Educate patients regarding preventive care, medications, lifestyle modifications, and disease management Participate in quality initiatives associated with a value-based care environment Work collaboratively with clinical and administrative team members to improve patient outcomes and continuity of care Qualifications MD or DO from an accredited medical school Board Certified or Board Eligible in Family Medicine or Internal Medicine Eligible for or currently holding an unrestricted Florida medical license Current DEA registration or ability to obtain one Strong clinical judgment and communication skills Commitment to high-quality, patient-centered care Interest in practicing within a value-based primary care model Experience with Medicare populations, chronic disease management, preventive medicine, or value-based care is beneficial but not required. About the Position This is an opportunity to practice primary care in an environment focused on quality of care, prevention, continuity, and long-term patient outcomes rather than traditional high-volume medicine. The practice is located in Jupiter, Florida and serves patients throughout the surrounding Palm Beach County community. Compensation and Benefits The position offers a competitive physician compensation package. Pay:
$240,000.00 - $270,000.00 per year
