Family Practitioner Full-time, Monday-Friday, 8: 00 AM-5: 00 PM Provides primary care including histories, exams, lab tests, prescriptions, and referrals Performs timely EHR documentation within 48 hours of each visit Provides patient education, counseling, and shared decision-making around diagnosis, treatment, and prognosis Coordinates care with internal and external providers and hospitals Prioritizes Value Based Care metrics at patient visits Attends site, department, and provider meetings May serve as collaborating physician for nurse practitioners, per collaborative practice agreement Completes medical chart reviews to evaluate standards of care and treatment outcomes May perform specialty services after additional training, credentialing, and certification

Requirements:
