Step into a vital nursing role dedicated to supporting patients on their journey to recovery in their own homes. This full-time, 13-week contract role is ideal for an experienced and compassionate Registered Nurse who thrives in an independent, field-based environment.

Schedule and Caseload:

Monday Friday, 8:00 AM 5:00 PM Caseload expectation: 2532 points per week (typically 35 patient visits per day) Travel required within assigned territory, with mileage reimbursement provided

Responsibilities:

Conduct comprehensive OASIS assessments and develop patient-centered care plans Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management Monitor and evaluate patient progress; promptly communicate changes to the care team and physicians Educate patients and families regarding conditions, treatments, medications, and preventive care Document care accurately and promptly using Home Care Home Base (HCHB) Collaborate with interdisciplinary clinicians for coordinated care delivery Participate in case conferences, staff meetings, and relevant continuing education Maintain compliance with regulatory standards and documentation requirements

Requirements:

Active RN license (in good standing) in the state of practice At least 1 year of recent home health experience (field experience preferred) Proficiency in Home Care Home Base (HCHB) and OASIS assessments Strong clinical assessment, organizational, and communication skills Reliable transportation, valid drivers license, and auto insurance Ability to work independently and handle a flexible field schedule

Preferred Qualifications:

BSN Experience managing 2532 weekly points Familiarity with Medicare and home health compliance
