Responsibilities:
Provide high-quality patient care to residents in our sub-acute medical care, rehabilitation services, and 24-hour skilled nursing care setting Conduct patient assessments, develop and implement individualized care plans, and collaborate with interdisciplinary teams to achieve optimal patient outcomes Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals Monitor patient progress, identify potential issues, and communicate effectively with patients, families, and healthcare teams Maintain accurate and timely records of patient care and treatment plans Demonstrate a commitment to patient-centered care, respect, dignity, and personal attention to the needs of each individual About the
