Sign On Bonus-$8,000.00 Prior experience not required. Ambassador will provide a thorough training program for RN candidates who do not have experience in MDS or ADON. Ambassador Health in Sidney, IA is looking for a candidate who can assist in directing and supervising our 46 bed skilled nursing facility team. This position is administrative in nature and will work directly with the Director of Nursing. The MDS Coordinator/ADON is responsible to oversee and coordinate the critical Minimum Data Set (MDS) process. You must be able to work closely with residents, their families, and staff on a personal basis. This key position is responsible for resident assessments and care plans in accordance with current rules, regulations, and guidelines that govern the Iowa MDS process. We are seeking candidates with the desire and willingness to work in a collaborative environment for the improved care and personal fulfillment of our residents.

Responsibilities and Duties:

Creating goals and treatment plans that focus on recovery and discharge planning of the resident and their families wishes Staff education regarding treatment plans and patient care Assesses and monitors the patient on a daily basis to promote patient recovery and provide quality care and safety Provides patient education on disease management, personal safety, and medication management Facilitates admissions and discharge planning with the patient and their family by coordinating with department heads of nursing, therapy, social services, nutrition services and the physician Supporting leadership role - alongside DON.

Qualifications and Skills Required:
