This isn't an entry-level role. We need someone who already knows how to manage a caseload, think on their feet in a patient's home, and work independently without a nurse's station down the hall. What you'll do Manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in the field Perform admissions, recertifications, and discharges, with accurate and timely OASIS documentation Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition Educate patients and family caregivers on disease management, medications, and home safety Respond to changes in patient status and escalate appropriately Maintain documentation that meets Medicare, state, and agency compliance standards What you need Active, unencumbered RN license in IL Minimum of [1-2] years of RN experience, with home health experience required Working knowledge of OASIS documentation and Medicare home health guidelines Comfortable working independently in patients' homes across [service area] Reliable transportation and valid driver's license Strong clinical judgment and the ability to prioritize a full caseload without close supervision Preferred Experience with Axes Certification in wound care, oncology, or chronic disease management Bilingual candidates encouraged to apply What we offer Competitive pay based on experience Supportive clinical leadership and a team that respects nursing judgment Room to grow with an agency that's investing in its clinical staff buqcQ5majq
